Identifying and addressing social determinants of health (SDoH) becomes imperative for healthcare providers to do their part in building healthier communities, extending beyond traditional medical interventions. This presentation will describe how our Division of Community and Population Health/Ambulatory Care leverages SDoH data to implement interventions that are atypical but critical to advancing population health and health equity. With creative approaches, others can also integrate these practices into traditional settings. Key ingredients include forging connections with community agencies, fundraising, coding maximization and strong advocacy at every level of the organization. Successful programming includes mobile markets for patients with food insecurity, peer referral services, pop-up vaccination centers, parenting support, mobile treatment buses and many more. We will facilitate discussion around designing interventions based on SDoH feedback and provide insight into our process, patient outcomes, successes and failures. The audience will receive a playbook of SDoH screening, reflect on related program design, and assess the feasibility of specific interventions.
